This physician will join Community Physician Network, a multi-specialty group with over 1,000 providers in Indianapolis, IN.

• Outpatient only
• Shared call schedule
• Patient Centered Medical Home practice model
• Fully Integrated EMR system (EPIC)
• Full onsite lab services
• Large group of specialists based nearby on the south side and at Community Hospital South

Licensure & Certifications :

• Board Certified or Board Eligible in Internal Medicine
• Licensed in Indiana or eligible for medical licensure in Indiana

Desired Requirements :

• Excellent communication skills fostering collaboration with all members of the care team
• Develop and maintain relationships with referring physicians
