As a key member of our Home Health Rehab team you will further our mission by :

  • Serving as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating physical therapy services to patients in their home.
  • Implement the care plan, including patient and family education, in collaboration with other team members.

For more than 30 years, Community Home Health has delivered excellent home care to patients and families in Fresno County and surrounding areas helping them make the transition from hospital to home.

As a part of Community Regional Medical Center, we are one of the Valley’s largest and most comprehensive programs, offering a full continuum of care, including Home Health and Home Infusion services.

Home care provides a welcome alternative to frequent and prolonged hospital stays.

Responsibilities

In this role some of your key responsibilities would include but are not limited to :

  • Evaluating, planning, organizing, and implementing specific treatment programs for individual patients.
  • Implements therapy according to established plan, utilizing appropriate procedures, equipment and techniques within the scope of physical therapy.
  • Educates the patient and / or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of treatment, home programs, injury prevention, and other related information.
  • Communicates effectively with other patient care team members and coordinates care provided.
  • Supervises PT Assistants with implementation of treatment plan.

Qualifications

Minimum requirements

Bachelor's Degree in Physical Therapy

State of California Physical Therapist License or eligible for licensure

  • Valid California Driver's License and driving record acceptable to Community Medical Centers required
  • Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)

Preferred qualifications :

1 year of experience in an inpatient or outpatient clinic setting
