We're seeking a detail-oriented and compassionate MDS Coordinator to join our vibrant team! In this crucial role, you'll play a vital part in ensuring our residents receive the highest quality care possible.
Key Responsibilities :
- Coordinate and oversee the completion of MDS assessments in accordance with federal and state regulations.
- Ensure accurate and timely submission of MDS assessments.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Monitor and evaluate resident care plans to ensure they meet the needs and preferences of each resident.
- Provide education and training to staff on MDS processes and documentation requirements.
- Participate in quality improvement initiatives to enhance resident care outcomes.
- Maintain up-to-date knowledge of MDS regulations and best practices.
Qualifications :
- Valid RN or LPN / LVN license in Wisconsin .
- 3 to 5 years of experience as an MDS Coordinator (or related field) in a long-term care setting.
- Comprehensive knowledge of MDS 3.0 guidelines and regulations.
- Strong analytical and organizational skills.
- Excellent communication and interpersonal abilities.
- Proficiency in electronic health records (EHR) systems.
- Ability to work independently and as part of a team.
