Schedule : Full Time, 40 hours per week Shift : Primarily Monday - Friday, 8am to 5pm, rotating weekends Location : Team Blue : This position will be conducting patient home visits in the West Longwood, Lake Mary, Apopka, Mt Dora, Sanford area which includes but is not limited to the following zip codes : 32757, 32777, 32798, 32712, 32768, 32703, 32779, 32746, 32771 The community you’ll be caring for : + Faith based and Mission driven agency + Flexible scheduling + One-on-one patient care in the patient’s home environment + Joint Commission Deemed Status accredited agency + Competitive pay with no state income tax + Commitment to workplace wellness and exceptional health and benefits offerings The role you’ll contribute : Provides vocational nursing services under the supervision of an RN, to patients in their homes.

The value you’ll bring to the team : + Performs nursing skills safely and competently. + Performs patient data collection in a holistic manner.

• Provides accurate and complete documentation. + Schedules daily visits to facilitate patient needs. + Plans patient care in consultation with Registered nurse.
• Assists with patient / caregiver teaching. + Demonstrates flexibility in response to unexpected changes and the ability to adjust priorities and use resources.
• Maintains open communication with RN Case Coordinator and RN Supervisor. + Demonstrates suitable time management skills.
• Meets agency's expectations regarding productivity. + Participates in monthly case conferences + Participates in agency Performance Improvement activities.
• Demonstrates good time management skills. + On call, as requested, RN as back up. + Provides orientation to newly hired LPNs and hosts student LPNs.

The expertise and experiences you’ll need to succeed : Minimum qualifications : + Licensed Practical Nurse or Licensed Vocational Nurse + BLS Certificate + Valid Driver's License and Auto Insurance + One year of medical / surgical experience Preferred qualifications : + One year experience in community health nursing + Thirty (30) hours of Florida Board approved IV Therapy Course This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
