Washington Regional Total Joint Center; and Washington Regional Pat Walker Center for Seniors.Position SummaryThe role of the RN Home Care Supervisor reports to the PACE Center Manager.
This position is responsible for coordinating the Home Care PACE program to ensure compliance with regulatory requirements while maintaining quality of program for participants.
This position promotes and maintains the health of enrollees in the community through teaching, counseling and appropriate preventive and rehabilitative services.
This position is responsible for the initial assessment of health needs, contributing to the plan of care and periodic re-evaluation of individual and family nursing needs.
Essential Position Responsibilities* Assess the home care needs of the elderly population to develop and implement specific plans of care including nursing, physical medicine, and personal care services, to ensure that quality service is provided to meet client needs* Coordinate durable medical equipment (DME) needs with participant* Actively participate in interdisciplinary care team meetings* Perform supervisory visits of home care technicians (HCT)* Partner with leadership team to manage and prioritize utilization of home care staffing resources to reflect participant home care needs* Develop programs and implement training materials and supervise in-home caregiver staff according to program expectations and regulatory requirements* Effectively communicate with participants and families regarding home care needs and resolve concerns appropriately* Document and maintain home care records, including service documentation, medical records, and billing and ensure accuracy* Maintain equipment, supplies, and services such as durable medical equipment, home delivered meals, medical response systems and incontinence supplies as reflected in the care plan* Provide in-home care to participant through prescribed treatments and conduct periodic assessment of participants to ensure care plan efficacy* Establish and maintain effective working relationships with program staff, agencies, and participants* Remain knowledgeable and current regarding PACE program regulatory requirements and program initiatives* Partner with leadership to develop and implement PACE policies and procedures, as neededQualifications* Education : Must be a graduate of an accredited nursing program* Licensure and Certifications : Registered Nurse license and CPR, required.
Experience : Minimum of 3 years of previous nursing experience. Previous experience with the elderly population, preferred.
Work Environment : This position spends 80% of time standing and / or walking while pushing, pulling, lifting, and / or carrying up to 50 lbs and 20% of time sitting while performing work in a standard office environment.
This position will require travel to participant homes. This position will serve a culturally and linguistically diverse patient population.
This position will be exposed to communicable disease and bodily fluids.Qualifications
