0 Guaranteed Hours : Information not provided Experience : Minimum of two years related experience in Health Care, with counseling and discharge planning experience License : Licensed Master Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) in New York State or eligible for New York State License Certifications : Screen certification or experience, or intent to pursue within six months Must-Have : Demonstrated ability to handle confidential information with discretion- Professional and courteous manner in dealing with people- Ability to meet deadlines, manage multiple priorities, and enhance team spirit through effective role modeling- Excellent interpersonal, communication, and organizational skills- Computer literacy- Experience with electronic medical records Job Description : Identify patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge- Provide high-quality, evidence-based care to patients while promoting cost-effective utilization of hospital resources- Collaborate with other interdisciplinary team members to develop and implement a comprehensive, integrated discharge plan for positive patient and family outcomes- Make appropriate referrals for community services and provide emotional support and therapeutic interventions if necessary- Offer crisis intervention, bereavement services, and patient / family education- Assess and intervene in situations requiring mandatory reporting- Supervise graduate social work student interns- Identify patients with non-emergent needs in the emergency department;
resolve barriers to accessing primary and preventive services- Educate patients about how and where to access healthcare services, including roles of PCP, urgent care, and emergency department- Refer to Health Home Care Manager and inform patients about available transportation resources in the community
