The RN Population Health Clinician is a professional RN whose role is to provide and coordinate care for Lake View Clinic (LVC) patients. The RN Population Health Clinician facilitates the attainment and maintenance of an optimal level of health for LVC patients by working directly with patients and families, providing care utilizing specialized knowledge, skills and abilities for a variety of ambulatory patient care needs. Independently implements the nursing process with patients / clients, performing assessments, monitoring and ensuring quality care through planning, observation and evaluation. The RN Population Health Clinician will act as supervisory backup to Clinic Director as delegated by Clinic Director or designee. The RN Population Health Clinician is also expected to participate and make recommendations regarding hiring, termination and disciplinary decisions and actions.

Education : Bachelor's Degree in Nursing.
Knowledge of the current practice standards that apply to practice as a Registered Nurse. Knowledge of equipment commonly used in a family practice setting, Knowledge of medical record documentation, customer and employee relations principles. Ability to use the telephone. Ability to write legibly, see, hear, speak English, and read. Working knowledge of computer processes appropriate to the functional area. Understanding of general health insurance principles and applicable billing guidelines and regulations. Professional verbal and written communication skills are required to establish and maintain effective working relationships with patients, physicians, team members, employees and the public. Ability to use analytical critical thinking and solid problem solving skills.
