Per Visit RN Home Health : Home Health and OASIS experience required.
Work any combination of days and hours : Monday-Friday 8a-4 : 30p and / or Sat-Sun 8a-4 : 30p or 8a-6 : 30p.
Requirements : Complete a minimum of 4 weekend visits each month and complete a minimum of 4 visits on any of the 6 designated holidays
Position Summary : Provides home health care to patients and families that focuses on preventing disease, promoting, maintaining or restoring health and minimizing the effects of illness or disability. Experience Preferred
- Minimum 1 year in acute care setting or comparable work experience.
- Home care experience
