About The Position

Responsible for the evaluation, planning, directing and administering of occupational therapy modalities of treatment as prescribed by a licensed physician.

Administers treatments and occupational agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability.

Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.

The staff therapist coordinates, delegates and supervises responsibilities assigned to supportive staff (OTA, Patient Care Assist.

etc.), and participates in scheduling patients, as well as all other aspects of patient care within the Department. The OT participates in operational aspects of the department, maintains performance improvement activities within the department and participates in QA activities.

Participates in all infection control, departmental equipment training, organizational safety and fire safety programs.DUTIES AND RESPONSIBILITIES : E Exceeds the Standard M Meets the Standard NI Needs ImprovementDemonstrates Competency in the Following of Patient Care and Application of KnowledgePerforms treatments and provides services to level of licensure and other duties as assigned within the LPN scope of practice.

210Able to provide detailed and timely patient report for the continuation of care and planning at shift change and upon handoff of care210Demonstrates the ability to assess patients of all ages to establish an individual POC for Occupational Therapy.

The Therapy Department treats patients of all ages, all debilities, including (but not limited to) pediatric, orthopedic, neuro, geriatrics and workers compensation.

210Evaluates, updates and revises individualized plan of care for each patient based on patient diagnosis and condition, individualized patient and family learning needs, and progressive goals for an effective transition of care and disposition210Knowledgeable the physiological basis for care delivery based on patient condition and / or diagnosis and institutes nursing actions based on the medical plan of care and accurate patient assessment210Notifies RN and / or provider timely of changes in patient condition and / or abnormal test results210Knowledgeable of common lab and diagnostic test results and able to relate abnormalities to condition and care delivery210Demonstrates proficiency in the initiation and / or management of peripherally IV s and central lines.

210Completes accurate and timely documentation of patient care status, medication administration, and clinical care activities210Able to organize patient care priorities and delegate tasks based on competency and licensure210Effectively communicates all procedures, treatments and medications to the patient and / or family210Provides for a safe patient environment to include call bell and personal belongings proximity, bed rail and bed position, clutter-free area, and frequent patient rounding for care needs210Ensures correct patient identification before any medication or treatment delivery by confirming the patient s identification band and utilizing two patient identifiers210Includes patient and / or family as part of the care team210Medication AdministrationAdministers medications safely and timely ensuring the rights of medication administration are followed210Demonstrates knowledge of medication purpose, dosages, side effects and contraindications210Safely administers, monitors and documents blood and blood components per policy210Able to recognize and take the appropriate actions for any medication or blood / blood component reaction210Professional and Personal ResponsibilityAdheres to the principles of good body mechanics when lifting and positioning patients210Completes incident reports timely to document any unusual occurrences210Reports to work on time and able to complete patient care activities within work shift without compromising quality210Adapts to changing workload as required210Assists in the orientation and integration of new employees210Recommends process improvement initiatives to improve patient care, quality and safety, and productivity210Knowledgeable of role in emergency / disaster plans210Complies with all isolation and infection control practices210Maintains all required certifications, licensure renewal and CEU s, and completes mandatory education210Maintains knowledge of and adheres to all hospital policies210Complies with all requirements of the Illinois Department of Public Health Licensing Act, Illinois Nurse Practice Act, OSHA, CMS and other regulatory and professional standards and hospital policies210Adheres to dress code, appearance is neat and clean210Wears identification while on duty, uses computerized punch time system correctly.

ACLS preferredSkills : Ability to read and communicate effectively in English. Basic computer knowledge Knowledge of blood borne pathogens, universal precautions, hazardous chemicals and fire and safety policies and proceduresWork Environment : Works in a well-lighted and ventilated office area, as well as throughout patient care areas.

Likely exposure with blood, body fluid, hazardous materials, radiation, and infectious diseases. Must be able to work beyond traditional work hours on all shifts and on weekends and holidays as needed.
