Amazing opportunity for a Registered Nurse seeking to transition into Case Management. Home Health agency will train high acuity nurses from emergency room or intensive care unit to become a Case Manager.

Rewarding role allows you to work independently and treat your patients. Good case load permits working in the field and in office without having to chart at home.
Monday Friday daytime hours and all travel time is included. CM provides a complete physical assessment, history of current and previous illness(es).
Regularly re-evaluates patient nursing needs, initiates the plan of care and makes necessary revisions as patient status and needs change.
Includes the patient and the family in the planning process. Collaborate with field nursing staff makes this role ideal.

Requires, Registered Nurse (RN). Will train high acuity nurses. Ideal candidate has experience working in case management and home health agency.
Candidates must have CPR and valid driver's license. Sharp communication skills and ability to interact with medical staff and patients.
Compensation 85K plus company car and tablet. Excellent benefits package includes 3 weeks of PTO with carry over, paid holidays, 401(k) and work / life / balance.
