About The Position
Job Title : RN Home Health Job Specialty : RN Home Health Job Duration : 13 weeks Shift : Day - 8x5 - 08A - 04P - 40 Hours Guaranteed Hours : Experience : Minimum 1 year License : Current licensure in the state of Massachusetts Certifications : N / A Must-Have : - 1 year of Certified Home Care experience - 1 year of Case Management experience - Reliable, independent means of transportation to make home visits - Willingness to work weekends / holidays / on-call in accordance with department needs / rotation (weekend rotations are once every 6 weeks) Job Description : - Provide clinical services to patients from birth to over 100 years in their homes.
- Assess patient conditions, implement care plans, and provide ongoing education and support to patients and their families.
- Perform the admissions process and case management responsibilities in accordance with policies and standards of practice.
- Utilize thorough and timely electronic documentation and engage in interdisciplinary communications. - Serve the communities of Ashland, Fayville, Framingham, Hopkinton, Hudson, Marlboro, Southboro, Sudbury, Waltham, Wayland, Weston.
