Alta Health Services is excited to announce a part time Occupational Therapist position for our home health division in beautiful Weber County, Utah.
We pride ourselves in the quality of care we provide, and we offer care options to suit every need. Our Occupational Therapist will help our patients with the following :
- Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of our patient’s physical and psychosocial functioning.
- Develop and revise the plan of care in consultation with the physician and other care-team members.
- Responsible for planning, evaluation, and treatment implementation of assigned caseload.
- Preparing clinical notes.
- Evaluating outcomes.
- Educate patients and / or families about proper safe use and recommend methods for installation of adaptive equipment.
- Candidate wil perform Oasis discharges
We are looking for a part time Occupational Therapist for our home health care division. We offer endless career potential, endless growth possibilities, and competitive compensation.
This position is based out of our Woods Cross, UT location at 576 West 900 South, Woods Cross, UT 84010.
Our Occupational Therapist will enjoy a competitive hourly rate of $43-$47 as well as a per visit rate ranging between $65-$70.
Our OT will receive an extra $10 per hour for drive time. Our Occupational Therapist will need a reliable vehicle, current auto insurance policy, and a valid driver’s license.
