Your Impact as a Stroke Program Nurse Navigator :
As a Stroke Program Nurse Navigator, you will :
Coordinate Multidisciplinary Care : Collaborate with medical staff, nursing staff, family members, and support services to provide comprehensive and coordinated care to stroke patients.
Deliver Stroke Education : Educate patients, families, and staff about stroke prevention, symptoms, and treatment options.
Provide Expert Consultation and Oversight : Offer expert nursing consultation and practice oversight to ensure the highest quality of care for stroke patients.
Manage Patient Care : Deliver and oversee patient care, including post-discharge follow-up, to ensure continuity of care and positive outcomes.
Collaborate with Key Stakeholders : Work closely with the Stroke Program Coordinator and other stakeholders to meet program certification requirements and improve the overall stroke program.
Drive Quality Improvement : Participate in data review and performance improvement projects to continuously enhance the quality of care provided to stroke patients.
Patient-Centered Approach : Incorporate the patient experience into all aspects of patient care, ensuring that each patient receives compassionate and individualized care.
Qualifications : Required :
Required :
Active California RN license
State of California Nurse Practitioner licensure (furnishing number in CA)
DEA furnishing certificate
BLS and ACLS certifications
NIHSS certification (within 3 months of hire)
ASLS certification (within 3 months of hire)
ENLS certification (within 6 months of hire)
CNRN or NVRN certification
Master's Degree in Nursing
Preferred : SCRN certification
SCRN certification
Strong ICU, neuro, and ED experience as a nurse (for new NPs)
