CLASS OVERVIEW
The Workforce Development Case Manager will support program participants in meeting their employment, career, educational and occupational skills training goals by providing direct case management services that will assist with barrier removal.
The Case Manager provides assessment,
planning, linkages to programs, coordination of services, monitoring of goals, evaluation of progress and advocacy. The Case Manager works as a team member with program staff and other professionals and services appropriate for each individual.
DUTIES AND RESPONSIBILITIES
- Provide quality Case Management services to all program participants.
- Establish and maintain a healthy and professionally appropriate relationship with all staff, program participants, partners, etc.
- Maintain confidentiality of all participants according to established policies, federal and state laws.
- Complete all assigned paperwork, including intake documents, according to program policies and procedures.
- Identify strengths, limitations, risk factors, and needs to refer and coordinate the provision of concrete services to each individual.
- Complete Individual Opportunity Plan (IOP) for each new participant as well as provide quarterly updates for all new and existing participants.
- Assist participants with making informed decisions about matters that are important to the completion of their identified goals.
- Assess to determine with participants’ needs are outside of the Youth Opportunity program’s scope of services and make appropriate referrals.
- Develop and maintain up-to-date case notes, and other appropriate documentation, based on contact with each participant in accordance with agency policy and procedures.
Attend and participate in professional development training for professional growth as developed by MOED / YO! Baltimore in addition to staff reviews.

EDUCATION AND EXPERIENCE REQUIREMENTS
EDUCATION : Have a high school diploma or GED equivalent.
KNOWLEDGE, SKILLS, AND ABILITIES
- Expert level written and verbal communication skills (English required?)
- Experience in project management
- Understand the concept youth development
- Very strong organizational skills and highly detail-oriented
- Very strong interpersonal skills and the ability to build relationships with a diverse group of stakeholders, including youth, staff, service providers, businesses, and other external partners
- Ability to effectively collect and analyze data, present findings and recommend viable options for addressing problems / challenges
- Demonstrated proactive approaches to problem-solving with strong decision-making capability
- Demonstrated ability to achieve high performance goals and meet deadlines in a fast-paced environment; and
- Proficient in word processing, spreadsheet, and database applications (i.e., MS Office Suite : Word and Excel) and project management applications
- Experience with Efforts to Outcomes (ETO) case management software preferred
