Primary Responsibilities :
- Care Plan Adherence and Execution :
- Follow a structured care plan based on each patient’s specific needs and conditions.
- Conduct home visits within assigned geographic areas to deliver personalized care.
- Medical Care and Monitoring :
- Administer medications, wound care, and other treatments or procedures as prescribed.
- Take and document vital signs, as well as monitor and report changes in patient condition to the Clinical or Client Services Manager.
- Maintain and execute doctors’ orders accurately, documenting all activities using a tablet device.
- Patient Assessments and Case Management :
- Perform regular assessments to adjust care as necessary.
- Coordinate with healthcare teams to ensure cohesive and efficient patient care.
- Direct Patient Support and Comfort :
- Provide empathetic care, assisting with feeding, bathing, catheter care, and ensuring clean bedding.
- Track and manage patient supplies, ordering additional items as needed.
- Build rapport with patients and families, offering emotional support.
- Communication and Documentation :
- Maintain precise documentation of observations, interventions, and evaluations for each patient.
- Engage in clear, compassionate communication with patients and family members, fostering trust and understanding.
Job Requirements :
- Education : Associate's degree in nursing required, Bachelor’s degree preferred.
- Experience : Minimum of 1 year in a nursing role within a residential or home care setting.
- Skills : Excellent communication skills, reliable transportation, and the ability to work independently.
- Attributes : Demonstrates empathy, compassion, and confidence in providing unsupervised nursing care.
