A full-time REHAB TEAM LEADER / Physical Therapist, (PT), position is now available in our sub-acute / skilled nursing Rehab Department.

Requirements
Qualifications :
Under the direction of a Rehab Director, a successful Rehab Team Leader will :
- Provide daily oversight of rehabilitation services
- Ensure efficient and effective operations of the facility's rehabilitation department
- Provide therapy services to patients
- Provide clinical support, training and oversight of specialty programs within his / her specific discipline.
- Plan and coordinate with other facility therapists in the implementation of programs and procedures to optimize patient outcomes
- Monitor patient admissions, schedules, MDS assessment periods and data
- Provide administrative supervision of therapists in the rehabilitation department
- Be a licensed Physical Therapist or license eligible in the state where employed
- Have experience as a treating therapist in the Skilled Nursing Facility setting.
