Preferred Qualifications : 1+ year of clinical experience in practice (long-term care setting preferred) Experience in meeting the medical needs of patients with complex behavioral, social and / or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficient computer skills including the ability to document medical information with written and electronic medical records Start & end times of shift : 8-5pm Days of the week they will work : Mon-Fri Dress code : medical attire Scrub color : n / a Travel questions Where they will travel : Loveland / Longmont, CO Larimer County - Specifically Longmont / Loveland, Brighton and Greeley area When will they travel : During Shift How many facilities will they be traveling between : 3-4 NP / Patient Connect : No.
