About The Position
Weekly Pay Range - $1627 To $1808 Job Title : Physical Therapist Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday-Friday, Day Shift Guaranteed Hours : 32 hours per week Experience : New Graduates are welcome;
SNF Experience preferred License : State License required Certifications : None specified Must-Have : COVID-19 test 72 hours prior to start date with results issued 48 hours prior Job Description : Ensure the prescreen is filled out and attached upon submitting the application.
- Attach resume and license verification for application consideration.- Conduct state license verification.- Provide three professional references.
- Complete criminal background checks (county, federal, and multi-state for the last 7 years).- Conduct a Nationwide Sex Offender Search (last 7 years).
- Perform HHS / OIG and GSA searches.- Provide Social Security Number Trace.- Submit 2 Step PPD or Quantiferon test results, or proof of Positive PPD and Chest X-Ray.
- Complete SAM Search and Pennsylvania State Police Background check.- Fill out the Communicable Disease Form.- Oregon State Background - CRIMS will be run by the facility.
- Review and comply with all credentialing requirements prior to submission. Note : Bilingual capability and holiday coverage are not required for this position.
