Requirements :
EXPERIENCE AND TRAINING
1. At least 16 years of age.
2. Active license as a Certified Nursing Assistant with State of WI
ESSENTIAL JOB FUNCTIONS
1. Maintains and actively promotes effective communication with residents / patients / clients and family members / personal representatives.
2. Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
3. Attend training sessions, staff meetings and in-services; complete required educational units annually and obtain a minimum of 12 hrs.

4. Read and adhere to resident rights agreement and report any violations immediately.
5. Treat residents, families and other staff with dignity and respect.
6. Use proper body mechanics in the performance of all tasks, maintain a safety focus.
7. Only perform tasks in which they have been trained; will not operate equipment without proper instruction.
8. Practice universal precautions and use personal protective equipment when dealing with blood and body fluids.
9. Maintain a calm composed attitude when dealing with difficult residents, families or staff.
10. Perform personal cares and mobility of residents as assigned and as directed in resident care plan.
11. Maintain confidentiality.
12. Maintain good attendance and take breaks only as assigned.
13. Work cooperatively with all disciplines.
14. Cooperate with scheduling clerk to accomplish adequate coverage for resident cares.
15. Document appropriately in the medical record and effectively utilizes electronic medical record.
16. Report to team leader prior to the end of shift.
17. Performs other similar duties, as needed.
