Job Summary
Join our PCHHC Family as a Home Health & Hospice Licensed Practical Nurse (LPN) , where you will play a crucial role in ensuring our patients receive exceptional care.
Our LPNs are responsible for verifying and executing the care plan as prescribed by physicians and under the guidance of a Registered Nurse.
This includes providing essential nursing treatments, promptly addressing patient pain, assisting with daily living activities, and educating patients and caregivers on self-care techniques.
They also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions.

Desired Skills & Qualifications
- Current Licensed Practical Nurse (LPN) license in state of practice
- Home Health and / or Hospice experience as an LPN preferred
- Ability to utilize full range of movement to transfer patients
- Effective verbal & written communication & time management skills
- Valid Driver's License (clean record) & reliable transportation.
- Proof of current auto insurance
- Ability to pass Criminal Background Check
- Current CPR Certification & Negative TB Test
