Mobile Family Nurse Practitioner
Job Summary
The Mobile Family Nurse Practitioner is responsible for providing comprehensive medical assessments and care coordination in non-clinical settings such as homes, nursing facilities, and pop-up clinics.
This role involves diagnosing medical conditions, ordering tests and procedures, and creating medical plans of care for various patient populations.
Job Duties
- Provide general medical care and care coordination for adults, women's health, pediatric, and geriatric patients
- Perform comprehensive evaluations and address chronic and acute primary care complaints
- Order diagnostic tests, seek specialty consultations, and prescribe medications as appropriate
- Participate in community-based Pop Up Clinics and build relationships with the community
- Drive to various locations within the assigned region to conduct in-person visits
- Collaborate with fellow nurse practitioners to develop best practices and participate in regional meetings
- Perform timely documentation in electronic medical records
Job Qualifications
Required Education : Master's degree in family health from an accredited nursing program
Required Experience / Knowledge, Skills & Abilities : Advanced computer skills, proficiency in Word, Excel, and Electronic Medical Record
Required License / Certification / Association : Active national certification, current state-issued license as a Family Nurse Practitioner, current Basic Life Support for Healthcare Professional certification, and unrestricted driver's license
Preferred Experience : 3-5 years of experience as a Registered Nurse and / or Nurse Practitioner, experience in home health or public health, and fluency in a language other than English
