Licensed Care Management Nurse / Social Worker

A company is looking for a Care Management Nurse or Social Worker.

Key Responsibilities

Collect and assess in-depth information about member situations to identify needs and develop specific care plans

Execute case management activities, integrating resources to achieve established goals

Ensure compliance with medical policies and standards of care while collaborating with internal and external stakeholders

Required Qualifications

Associate or Bachelor's Degree in Nursing, Behavioral Health, or a related field

3 years of experience in case management, utilization management, or behavioral health case management

Must have licensure or certification in a relevant health or human services discipline

Experience in direct clinical care for at least 3 years

Familiarity with health insurance industry trends and healthcare documentation systems
