Overview :
We are seeking a capable and organized Nurse Practitioner with strong clinical skills to join our team of providers. We are an Internal Medicine division of a large multi-specialty group, seeking an independent, capable and kind Nurse Practitioner, preferably with a special interest in the geriatric population, to join our Practice.
Our candidate must be independent and willing to travel between local facilities. Candidate should be able to provide direct patient care in a Long Term Care / Skilled Rehabilitation or Assisted Living setting, prioritize tasks and work independently.
Excellent written and verbal communication skills; basic computer skills, demonstrate organizational skills and multi-tasking required.
While experience is preferred, we are willing to train the right candidate. Salary will be determined commensurate with experience.
All team members participate in our practice bonus structure, with extensive benefits provided by the larger practice group.
Responsibilities :
- Conduct patient assessments, including physical exams and medical histories, to develop accurate diagnoses.
- Diagnose and manage acute and chronic health conditions, prescribing medications as needed.
- Educate patients on health management and preventive care.
- Collaborate with physicians to develop and adjust personalized care plans.
- Maintain detailed and accurate patient medical records, ensuring treatment effectiveness.
Qualifications :
- Advanced nursing degree (MSN or DNP) with RN and Nurse Practitioner licensure.
- Proven experience in clinical settings, with strong diagnostic and patient care skills.
- Excellent communication, problem-solving, and teamwork abilities.
