SUMMARY :
The LPN personally administers care to assigned mid-teen to adult and geriatric physical medicine rehabilitation patients from adolescent age upward under the direct supervision of a registered nurse.
The individual is responsible for documentation and a myriad of tasks as outlined.
ESSENTIAL FUNCTIONS :
Observes and documents conditions / behaviors / needs of the rehab patient for initial patient assessment. Complete data is obtained and documented by physical observation, interviews with patient and significant others, review of records, review of age-related changes, etc.
within the established time frame. Documents consistently in the electronic medical record (which includes system review, mini templates regarding restraints, bowel, bladder, etc.).
- Performs medical care tasks as necessary for assigned team. Administers medications, monitors and administers IV's, etc.
- assuring completeness of ordered procedures and treatments and instructing patients concerning procedures, treatments and needs;
medical orders are carried out accurately and on time. Understands and follows patient care plan; input concerning changes to patient care plan is communicated based upon own observation.
Monitors patients according to patient care plan; identifies, communicates and documents changes according to established procedures.
Reinforces patient education needs and documents in the electronic medical record.
Administers personal care to patients, including bath and skin care. Patients' personal hygiene and basic needs are met in caring and professional manner.
Demonstrates appropriate sterile, aseptic and hygienic technique to prevent the spread of infection to patient, self and others.
Incorporates universal precautions, hazardous chemicals, and infectious waste policies into patient care.
The incumbent completes formal ongoing competency evaluation successfully, maintains and demonstrates an acceptable level of clinical competency in the care of the physical medicine rehabilitation
population served from adolescent age upward

EDUCATION AND EXPERIENCE
- Current LPN license (current and unrestricted)
- Graduate of NLN accredited Licensed Practical Nurse Program required
- Experience as LPN in hospital setting preferred
- Experience in rehab setting is preferred
HOURS : 3-12 hour shifts a week; every third weekend; 7 : 00 AM-7 : 30 PM
