Job Description :
Supplemental Health Care is seeking Social Workers in Albuquerque, New Mexico with prior OASIS / Medicare Certified Visits for contract assignments with a partnering home health agency.
Home Health Social Worker Contract Details :
- $1, - $1, per week*
- SHC Home Health Social Workers are paid by the hour, not by the visit
- Social Worker will have a designated territory of coverage
- Weekly expectation is 30 units of productivity
- Home Health contracts with SHC require previous experience with OASIS / Medicare Certified Visits
- Estimated weekly payment may include both taxable wages and tax-free reimbursements for meals, housing, and incidentals.

Requirements :
- OASIS / Medicare Certified Visits Experience
- Electronic Charting experience
- Minimum of 1 year of recent home health Social Worker experience (private duty does not qualify)
- Most contracts require some on-call
- Current New Mexico Social Worker License
- American Heart Association BLS
- Valid U.S. Driver's License
