• Case Details : Various ages of children ages birth thru 30+years old with technologies that might include g-tube, trach and ventilator.

Responsibilities

• Contributes to the development, review, and revision of the care plan.
• Administers nursing care in accordance with the physician's treatment plan and the individual care plan authorized by the client's payer source.
• Observes, documents, and reports the client's response to treatment and any changes in their condition to appropriate personnel and / or physician.
• Provides ongoing guidance and supervision to the client, family, or other healthcare team members involved in the treatment plan.
• Monitors and documents the patient's condition and the care services provided through electronic charting.
• Participates in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.

Requirements

• Active Licensed Practical Nurse or Registered Nurse License
• Problem solving skills and ability to multi-task
• Excellent Communication Skills
• Positive, Professional attitude
