Registered Nurse Case Manager

Remote Position, but the candidate needs to be from the Northeast area of OH - Will be working in the following counties : Cuyahoga, Medina, Lorain, Lake, and Geauga
Job Summary :
- Daily Responsibilities :
- Outreaching to new enrollees to complete initial assessments and care planning.
- Carry average of 40 new members each month
- Performance Expectations / Metrics :
- Be able to work independent

- RN with case management experience
Pay : $35-$45 hourly (determined on experience)
Work Schedule : Will be 40 hours week from 8am-5pm Monday - Friday
Required Education & Experience :
- RN active license in good standing with the Ohio state board of nursing is required.
- 1-3 Years of Case management experience with developing a Plan of Care
- Knowledge of computer skills : Type at least 45 wpm, and have skills with MS Word, Outlook, & Excel
Preferred Experience :
- 1-2+ years of utilization review experience.
- Case Management certification (CCM) is a plus.
- Tru Care experience Is A Plus
