VNA Health Group is seeking a Registered Nurse (RN) Case Management for a nursing job in Englishtown, New Jersey.

Overview :

Responsible for providing assessment and referrals for individuals living with HIV to increase access to medical care and other needed resources.

Responsibilities :

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Performs assessment of individual needs and develops and documents appropriate plan of care
• Anticipates insurance lapses, assists with required insurance and medication coverage applications
• Assesses for related program eligibility, mental health, housing services, substance use services, and other entitlements
• Documents all required data into the reporting systems including CareWare. Provides reports as requested by Program Manager
• Provides ongoing follow up and evaluation of the effectiveness of services being provided
• Adheres to the procedures and documentation as required by Part B Standards of Care for Case Management
• Adheres to agency infection control and safety policies
• Adheres to the organization’s policy in regards to absenteeism and appearance

Qualifications :

• Bachelor’s degree in Social Sciences or related field
• 1+ years of HIV / AIDS work related experience
• Knowledge of cultural and lifestyle diversity issues
• Strong organizational and interpersonal skills
• Bi-Lingual Spanish preferred
