About The Position
Job Title : Occupational Therapist Job Specialty : Home Health Occupational Therapist Job Duration : 13 weeks Shift : Days - Monday to Friday, 8-hour shifts Guaranteed Hours : 40 hours per week Experience : Home health experience preferred License : Occupational Therapy License Certifications : Relevant certifications as required by the state Must-Have : Experience with home healthcare systems Job Description : Deliver comprehensive occupational therapy services to patients in their homes.
- Develop individualized treatment plans to improve patients' abilities to perform daily activities.- Collaborate with healthcare team members to plan and execute effective treatment strategies.
- Provide guidance and support to patients and their families to enhance recovery.- Utilize electronic health records system, experience with HCHB preferred.
- Participate in case conferences and team meetings as needed.- Ensure compliance with all regulatory and company standards and policies.
