Coordinate services to Health and Welfare clients both directly and through engaging with partnering service providers, churches, volunteers, and community organizations to address areas of vulnerability and facilitate stability and self-sufficiency.
ROLE & RESPONSIBILITIES :
Provide Intensive Case Management for select refugee clients who's specific vulnerabilities hinder their ability to integrate into the community and achieve self-sufficiency.
This includes the following responsibilities :
- Conduct pre- and continued screening with each client referred to the Preferred Community (PC) program to determine eligibility and conduct follow-up assessments at the 90th, 180th, 270th, and 360th day to monitor client progress
- Guide the goal-setting process with each enrolled client to identify areas of vulnerability and detail action steps necessary to promote self-sufficiency
- Assess family units from an ecological and solution-oriented perspective
- Perform regular home visits and welfare evaluations throughout the program period
- Identify community resources that best address client needs
- Ensure provision of all necessary services as outlined by the program guidelines, to include accompanying clients to appointments (medical, legal, DSS, etc)
- Build a comprehensive self-sufficiency plan with clients to include ESL, Employment, Housing, Education, Medical, and Socio-Cultural integration
- Coordinate cross-departmental internal referrals and services at World Relief
- Maintain accurate documentation of client files, case notes, reporting records, and submit monthly program reports
- Provide mediation services between clients and outside entities (i.e., healthcare providers, volunteers, etc), and additional adjustment counseling as needed
- Ensure professional and timely provision of all necessary services as outlined by the PC Guidelines
- ADMINISTRATIVE AND PC PROGRAM RESPONSIBILITIES
- Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
- Submit any requested data to the program manager through monthly, semi-annual, and annual reports
- Adhere to all World Relief policies, including but not limited to, incident reporting procedures, code of conduct, security safety and health
- Perform other tasks and projects as assigned
JOB REQUIREMENTS :
- Mature and personal Christian faith
- Committed to the mission, vision, and values of World Relief
- Desire to serve and empower the Church to impact vulnerable communities
- Able to affirm and / or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Evangelicals For the Health of The Nation document
- MS Office skills required
- Highly self-motivated with strong follow-through and problem-solving skills
- Valid driver's license, regular access to vehicle, and ability / willingness to drive clients to appointments
- Desire to serve refugees in support of the overall mission of World Relief
- Excellent organizational skills with attention to detail
- Excellent written and oral communication skills in English
- Cultural competence
- Ability to navigate multiple systems (health care, housing, DSHS)
- Able to work under pressure and prioritize tasks
- Intermediate MS Office knowledge and skills
- Flexibility with willingness toward new learning
- Self-motivated worker, able to demonstrate initiative
PREFERRED QUALIFICATIONS :
- Bachelor's Degree in Social Work or a related field
- 2-years work experience in a helping profession
- Demonstrated life experience may be considered in lieu of education or work experience
- Bilingual candidates preferred
- Direct-service experience with vulnerable populations preferred
- Cross-cultural experience strongly preferred
$19.50 - $20 an hour
Compensation is commensurate with experience.
