Social Worker

Position Summary :

Florida Atlantic University is seeking a Social Worker - Boca Raton, FL.

This position is an integral member of an inter-professional healthcare team providing services and supportive care at the Louis and Anne Green Memory and Wellness Center of the Christine E.

Lynn College of Nursing at Davie in Broward County. All clinical services are provided in accordance with established standards of practice and ethical professional conduct including adherence to HIPPA policies for protecting privacy and confidentiality, and in alignment with the philosophy of caring of the Christine E.

Lynn College of Nursing. This position is under the supervision and in collaboration with the Director of the MWC.

Summary of Responsibilities :

Conduct informant and / or supervise patient / family interviews.

Participate in patient / family case conferences.

Consult with treating providers as needed.

Provide educational opportunities for students of multiple disciplines, including didactic lectures and field instructions for social work students.

Provide educational lectures to patients, families and the community at large.

Provide in-service training to staff of the Memory and Wellness Center.

Facilitate and expand family caregiver support groups.

Identify grant opportunities and assist with content development.

Provide memory screenings to the community, as needed.

Conduct home visits; develop care plans; ensure ongoing care coordination of individuals living alone with dementia; examine unmet needs, social isolation, home safety / risk, medication management.

Identify existing or potential caregivers to expand support available to those who are living alone without a caregiver.

Other duties as assigned.

Minimum Qualifications :

Master's degree from an accredited institution in Social Work; or a bachelor's degree from an accredited institution in Social Work and two years of appropriate experience required.
