Title : Home Health Care Nurse

Job Description :

We are seeking a compassionate and skilled Home Health Care Nurse In Melbourne Area to join our team. As a Home Health Care Nurse, you will be responsible for providing personalized medical care and assistance to patients in the comfort of their own homes.

Your primary goal will be to ensure the well-being and safety of our patients while promoting their independence and quality of life.

Responsibilities :

Conduct comprehensive assessments of patients' health conditions and develop individualized care plans
Administer medications and treatments as prescribed by physicians
Monitor patients' vital signs and maintain accurate medical records
Collaborate with other healthcare professionals to coordinate patient care
Educate patients and their families on proper self-care techniques
Provide emotional support and guidance to patients and their families
Assist with activities of daily living, such as bathing, dressing, and grooming
Regularly evaluate patients' progress and adjust care plans as needed
Ensure adherence to infection control protocols and maintain a clean and safe environment

Qualifications :

Valid nursing license in Florida
Proven experience as a Home Health Care Nurse or in a similar role
Excellent knowledge of nursing principles and practices
Strong communication and interpersonal skills
Ability to work independently and in a team environment
Compassionate and patient-centered approach to care
Attention to detail and excellent organizational skills
Proficient in electronic medical record (EMR) systems
Valid driver's license and reliable transportation
