We are seeking a Registered Nurse Case Manager to join our Home Health team. In this role, you will provide comprehensive home health services to clients within their place of residence, including assessments, interventions, and supportive care.
You will work autonomously, build strong rapport with patients and families, and manage your schedule flexibly. Our ideal candidate is a Massachusetts RN with at least one year of acute medical / surgical nursing experience and CPR certification.
