Memory Care Program Coordinator

We are hiring a Memory Care Program Coordinator to join our recreation team at Worcester Rehabilitation & Health Care Center!

40 hours a week, Monday-Friday 8a-4p with weekly pay.

Experience & Education :

  • Must possess a high school diploma or its equivalent.
  • One (1) year experience in a supervisory capacity in a hospital, skilled nursing care facility or related medical facility activities department is preferred.
  • At least one (1) year experience or training in directing group activity.
  • Experience working with those diagnosed with Dementia or Alzheimer’s is required.
  • Certified Dementia Practitioner (CDP) required.

Duties & Responsibilities :

  • Assist in plans and provisions of patient care by visiting residents regularly to evaluate needs.
  • Work alongside residents with dementia and Alzheimer's and instruct activities and events specific to their needs.
  • Document and report to the nursing department any unusual or significant findings.
  • Encourage residents to take part in activities of daily living that will stimulate their potential, physically, mentally, socially, and spiritually.
  • Spend duty time with residents rather than with other facility staff.
  • Assist with and follow through on the establishment of resident care goals within the framework of the entire therapeutic team.

Specific Requirements :

  • Sensitivity to those diagnosed with Dementia and their families.
  • Understanding of appropriate activities for those with dementia.
  • Ability to read, speak and understand the English language.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully with personnel, residents, family members, visitors, and government agencies.
  • Knowledgeable in the Privacy rule and ensure all staff comply with facility privacy practices policies and procedures.
  • Leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel.
  • Patience, tact, cheerful disposition, and enthusiasm, as well as willingness to handle residents based on their maturity level.
  • Ability to seek out new methods and principles and incorporate them into existing nursing principles.
  • Ability to relate information concerning a resident’s condition.
  • Participation in continuing education and inservice training.
