Responsibilities
Manages the daily operation of the Case Management Department staff to provide effective and efficient discharge planning services to patients in accordance with policies, procedures, regulatory requirements consistent with the mission, vision and values of the Jefferson Health Enterprise.
Essential Functions
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Supervises the Department staff at identified hospitals.
- Responsible for the recruitment, hiring and onboarding of all new staff.
- Responsible for updating Departmental policies and procedures, including an annual review of all departmental policies and procedures.
- In conjunction with the Director, collaborates with Hospital Leadership team to establish and implement departmental functions and goals in accordance with both departmental, hospital and enterprise strategic initiatives and goals.
- Actively participates in relevant hospital administrative meetings representing the Department.
- Responsible for completing / participating in completion of annual employee performance evaluations of the staff.
Qualifications
Education :
BSN required. MSN or MBA preferred.
Experience :
Minimum of 5 years’ experience in hospital based Case Management experience required. Minimum of 2 years prior management experience required.
Minimum Certifications, Registration or License Requirements :
CCM preferred; active relevant Registered Nurse license in Pennsylvania / New Jersey.
Conditions of Employment
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check.
The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
Conditions of Employment
Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson’s clinical entities or at the University.
If you are not currently vaccinated you will be required to receive the vaccination prior to hire date if you are offered employment, unless you request and receive an approved medical or religious exemption from Jefferson.
