Responsibilities and Duties
- Maintains direct and frequent contact with residents particularly during higher-risk activities including mealtime and bath times.
- Provides routine nursing care for all residents.
- Obtains and accurately transcribes physician's orders and any changes in orders as situations occur.
- Updates any changes in medication orders in a timely manner in the EMR system.
- Administers and documents medications administered as ordered by a physician.
- Performs controlled medication shift count with incoming Nurse / Med Technician and reports any discrepancy to Director of Nursing.
- Makes necessary entries in daily nursing notes, Therap, condition sheets, and wound tracking forms to reflect the care and condition of residents, completing monthly summaries / quarterlies as assigned.
- Conducts shift reports in a professional, organized manner so that all pertinent information is communicated to the on-coming nurse.
- Communicates with physician and Director of Nursing (DON) any significant medical problem or change in condition.
- Maintains a check of medical and health-related supplies and informs the MTA / DON of any items that need to be ordered prior to the complete exhaustion of those items.
- Orders medications and treatments according to the Pharmacy Policy and Procedure Book.
- Maintains effective communication and planning efforts with the DON, QIDPs and other personnel.
- Functions as an integral part of the Interdisciplinary Team.
- Responds proficiently to emergency situations and other unusual circumstances.
- Notifies the Administrator On-Call when situations arise that need clarification.
- Assists Disability Support Professionals as necessary.
- Provides effective communications and work relationships with shift employees.
- Provides in-service training and staff competency checks as needed and directed.
- Assists with preparing client daily formulas if needed.
- Documents and changes in condition, PRNs given and works with the physician to provide any needed nursing care for all clients.
- Assists the Medication Technician / Group Home Manager / DON in making any urgent medical appointments for the Arches clients.
- Performs other duties as necessary and / or assigned.
Qualifications and Skills
- Must hold a current valid license to practice as a Licensed Practical Nurse (LPN) in the State of North Carolina
- Prefer at least one year of pediatric or long term care experience with knowledge of ICF (intermediate care facility) helpful
- Must possess excellent written and oral communication skills
- Proficient in electronic medical documentation
- Preferred certification in Adult and Pediatric CPR, First-Aid, AED, and BLS
- Valid NC driver's license
