Licensed Clinical Social Worker (LCSW) - Case Management

Location : Palm Springs, CA

Virtus Healthcare Staffing is seeking a dedicated LCSW to join our partner hospital's Case Management team in Palm Springs, CA.

This role focuses on facilitating patient care transitions, optimizing resource utilization, and ensuring patient well-being throughout their healthcare journey.

Responsibilities :

• Transition Management : Assess and address patients' needs for smooth transitions across the care continuum, promoting appropriate length of stay, preventing readmissions, and enhancing patient satisfaction.
• Care Coordination : Collaborate with the healthcare team to ensure efficient throughput and timely delivery of care at the appropriate level, prioritizing patient safety and quality.
• Psychosocial Assessment and Intervention : Conduct complex psychosocial assessments and provide interventions to support patients and families facing emotional, social, and practical challenges related to their health condition.
• Compliance and Education : Ensure adherence to internal policies and state / federal regulations related to discharge planning and social services.

Educate staff, patients, and families on relevant topics.

Case Management : Participate in complex case reviews and daily patient care conferences. Oversee case management and keep providers updated on any changes to post-discharge plans.

Qualifications : Required :

Required :

• Master's Degree in Social Work
• CA LCSW License
• Excellent verbal, written, and computer skills
• Knowledge of internal policies and State / Federal regulations related to discharge planning and social services
• 5150 certification within 90 days of hire and maintained at all times

Preferred :

2 years of hospital or healthcare experience

Physical Requirements :

• Regularly required to sit, talk, and hear.
• Frequently required to use fine motor skills (typing / data entry) and reach with hands and arms.
• Frequently required to stand and walk; occasionally stoop, kneel, or crawl.
• Must regularly lift and / or move up to 20 pounds and occasionally lift and / or move up to 50 pounds.
• Works in a clinical and office environment.
