Position Title : Social Worker (LSW)
Schedule : Monday-Friday 8am-5pm
Pay : $25-$30 hourly

Who you are :
- Bachelor's degree in social work required; Master's Degree in Social Work / LCSW preferred.
- At least two years of appropriate professional experience in geriatrics, social work, or health care.
- Must have effective communication skills, both oral and written.
- The ability to promote a courteous, calm atmosphere.
- Be able to work cooperatively with other healthcare professionals and other departments within the system.
- Show the ability to demonstrate initiative by actively seeking additional duties, as well as accepting and performing all other duties as assigned.
General Summary :
The Social Worker helps their patients / residents identify their concerns and then suggest solutions or identify resources that may help their patients / residents.
Essential Job Duties :
1. Provide individual, family, and group mental health counseling on the Carillon campus.
2. Consults with physicians and nursing administration on treatment program for resident.
3. Monitors medication compliance.
4. Meet regularly with Care Plan Group to discuss resident's care plan.
5. Develop social histories or residents and develop social assessments and care plans for each resident that identifies medically related social and emotional problems and needs.
6. Promotes and interprets Residents Rights.
7. Coordinates admission process for new or returning residents.
8. Coordinates discharge planning with staff, residents, and families.
9. Completes all necessary and supportive documentation-progress notes, intervention notes, etc.
