JOB TITLE : LPN / RN

DEPARTMENT : Nursing

REPORTS TO : Nursing Supervisor

Position Summary

The LPN / RN provides direct personal care services to Residents in the home and assures that all company policies and procedures, standards and Residents rights are observed during his / her assignment in the home.

Position Requirements

  • Adhere to the Resident Assessment and Care Plan for each of the Residents in the home.
  • Provide skilled personal assistance with bathing, dressing, oral care, grooming and any other assistance needed to assure that all personal needs are met completely, in a timely manner.
  • Provide dignified assistance for incontinent residents.
  • Administer medication to Residents in accordance with State regulations and training standards, assuring that all medication charts are kept up to date.
  • Maintain an accurate Narcotic Count following the policies and procedures of the company. Any discrepancies will need to be communicated to the Nursing Supervisor immediately.
  • Assess within your scope of profession (falls, skin tears, wounds, emergency situations, etc.) and communicate with residents DPOA / Emergency Contact and Doctors.

Report end results to Nursing Supervisor and / or Operator, with proper documentation within nursing notes; including assessing the situation and determining the cause.

  • Assurance that all Resident care logs, charts, records, journals, etc. are kept current.
  • Be knowledgeable at all times regarding Resident behavior problems; effect and effectiveness of medications; changes in behavior, appetite, sleeping habits;

doctor's appointments; family outings; and other pertinent information. Document any significant events in nursing notes.

Notify On Call or Nursing Supervisor, as determined necessary.

  • Communicate with CMAs on medication changes, discontinued medications and behaviors.
  • Communicate with CNAs on Resident Care changes.
  • Keep the home clean, orderly, organized, and safe at all times, and does the laundry of the home and the Residents on a regular basis.
  • Prepare meals according to the menu plans (including special dietary needs) and follow the daily schedule.
  • Provides focused individual Resident interaction and activities in accordance with each Resident's Assessment and Care Plan, assuring appropriate movement, exercise and mental stimulation for each Resident in the home.
  • Is knowledgeable of KDADS care regulations and compliance requirements.
  • Interact with Residents, families, and other staff in a professional manner.
  • Participate in all required staff education meetings.
  • Perform other reasonable duties as requested by Management.
  • Monitor over condition of the property including yard, upkeep, trash removal, snow removal, etc. and refers all home or yard maintenance problems to the Management.
  • Act in the best interest of Home of the Flint Hills at all times.
  • Adheres to company appearance guidelines always when working in the home, attending a staff meeting or training session, or representing the company in any other capacity.
  • Become knowledgeable of all the Company's policies and procedures.

Position Requirements

Qualifications :

Currently certified and in good standing by the State of Kansas as a Licensed Practical Nurse or currently certified and in good standing by the State of Kansas as a Registered Nurse.

Successful completion of HOTFH Orientation and Training Programs.

  • Pushing / Pulling a minimum of 50 pounds (i.e. moving furniture)
  • Standing for long periods of time (min. 1 hour, i.e. passing meds, washing dishes, cooking, showering assistance)
