Job Summary

The Ambulatory Clinical Registered Nurse (ACRN) provides nursing care to assigned patients in an outpatient facility or clinic.

Records patients histories, performs various diagnostic tests, and develops patient care plans in conjunction with other medical professionals.

The ACRN is an energetic, organized individual who takes ownership and initiative in collaborating with providers, leadership, and other clinical staff in assisting in the coordination of care for patients as a member of the patient care team.

  • These duties will include : 1) Promoting nurse-sensitive quality indicators by providing guidance and oversight of the clinical work performed in the ambulatory care setting;
  • 2) Utilizing evidence-based information across a variety of outpatient health care settings to achieve and ensure patient safety and quality of care while improving patient outcomes, which includes clinical, organizational, and professional activities with and for individuals, groups, and populations needing improved health or care for health-related problems;

and 3) Promoting of optimal wellness, participating in the management of acute illness, and assisting the patient in managing the effects of chronic disease and disability.

The ACRN will be responsible for facilitating continuity of care using the nursing process, interprofessional collaboration, and coordination of access to appropriate health care services and community resources across the care continuum.

The ACRN will also assist with identifying opportunities for collaboration with the QUEST transformation team in order to improve processes that promote patient safety.

Minimum Job Qualifications

Licensure or other certifications : Active Georgia licensed Registered Nurse. Competency must be obtained through experience and / or certification.

Essential Tasks and Responsibilities

Perform patient assessments. Review and / or collect pertinent patient information (i.e., medical history, medications, etc.

and document all care and interventions in the patient's medical record.

Applies appropriate nursing interventions, such as identifying and clarifying patient needs, performing procedures, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient in navigating the health care system, and evaluating patient outcomes.

Prepares and administers medications, IV fluids, other complex treatments prescribed by the provider within the RN's scope of practice.

Act as a partner, advocate, and advisor in assisting and supporting patients in the optimal management of their health care, respecting their culture and values, individual needs, health goals, (based on educational level) and treatment preferences.

Competent in complex decision-making. Discusses complex patient needs with provider(s) and clinical staff. Creates care plans, documents patients needs, and coordinates services including the planning of appropriate nursing care and the delegation of appropriate tasks to ancillary staff.

Demonstrates accountability across the care settings in maintaining continuity of care and is proficient in documentation in the electronic medical record.

Facilitates patients' and / or populations' progress toward positive person-centered clinical outcomes.

Utilizes an interprofessional approach to engage patient, caregivers, and providers in implementing the plan of care across care settings.

Facilitates the transition of patient / populations to the appropriate level of care.

Provide education to patients and caregivers incorporating principles of health literacy and cultural sensitivity.

Implements nursing-initiated protocols, monitors the status of chronic care populations, and collaborates with the interprofessional team to improve outcomes.

Demonstrates ability to successfully navigate patient care coordination and transition of care management practices for a wide range of patients based on chronic conditions and co-morbidities, risk of hospital admission or readmission, including successful billing of incident-to services.

Competently provides assessment services for eligible components of annual wellness exams, preventive health screenings, evidence-based protocols, completion of value-based purchasing, ACO and other metrics, or other services, including incident-to services, allowable by law and licensure.

Services are provided face-to-face, via telehealth, or other acceptable environments.

Provides initial and follow-up education to patients via direct interaction in the office setting for new diagnoses and uncontrolled chronic medical conditions.

Also, documents patients response to education provided to the patient while identifying barriers and solutions to compliance.

Is able to bill for educational services offered to patients.

Collaborate with providers to determine follow up plan of care required to manage disease process effectively.

Reduces admission, readmission, and ED utilization rates by identifying those patients at moderate to high risk of utilizing these services and collaborating with other members of the patient's care team on interventions.

Assists in closing care gaps by identifying, delegating, and / or performing necessary tasks required.

Partner with patients, families, and caregivers to set goals of therapy and promote accountability for self-management.

Recognizes and maximizes opportunities to increase the quality of care. Communicates relevant information to patient, caregivers, and interprofessional health care team across the care continuum.

Assists in research methods to assess the quality of services directly provided and effectiveness of the ACRN role.

Mentors, orients and coaches others in unit specific operations and patient care activities.

Serves as leader of clinical activities for the health care team; delegates tasks appropriately and demonstrates appropriate accountability.

Understands own and team members' scope of practice and escalates issues as appropriate.

Assists with Annual Wellness Visits as determined by CMS Guidelines & Physician Protocols.
