Overview :
Per Diem RN Responsible for providing skilled nursing care to patients of the Mobile Outreach Clinic Program
Responsibilities :
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Provides case management for program clients
- Monitors and ensures quality and appropriateness of services provided are in accordance with the agency and contract guidelines
- Provides completed documentation in accordance with agency guidelines
- Develops and provides individual and group education for patients and site staff
- Establishes and maintains community relationships to support client referrals
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance.
Qualifications :
- RN licensure for New Jersey
- 2+ years of community health nursing experience; bilingual preferred
- Strong Computer skills
- Excellent verbal and written communication skills
- Excellent interpersonal skills with demonstrated ability to communicate with outside personnel
- Ability to travel with a valid NJ driver’s license required
