Kingston Healthcare is seeking a compassionate and dedicated Social Worker to join our team of healthcare professionals. As a Social Worker at our facility, you will be responsible for providing social services to residents and their families to ensure their emotional and social well-being.
Responsibilities :
- Conduct psychosocial assessments of residents and develop individualized care plans.
- Provide counseling and support to residents and their families.
- Collaborate with the interdisciplinary team to coordinate care and services for residents.
- Assist residents in accessing community resources and services.
- Advocate for residents' rights and well-being.
- Maintain accurate and up-to-date documentation of all social work services provided.
Qualifications :
- Bachelor's or Master's degree in Social Work from an accredited program.
- Current Social Work license in the state of Ohio.
- Previous experience working in a healthcare or long-term care setting preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively as part of a team.
