Job Title : Discharge Planner / Case Manager

Shift : Day 5x8-Hour (08 : 00 - 16 : 30), with day and rotating weekend shifts

License : Must have a DC License in hand. Social worker must be eligible for LMSW / LGSW / LCSW and have passed the exam.
Certifications : Basic Life Support (BLS), RN or LMSW / LGSW / LCSW
Must-Have :
- Experience in discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters.
- Experience with the following work setting skills : Cerner, Acute Hospital, Insurance Company, Long Term Acute Care / Rehab / Skilled Nursing.
- Regulatory skills involving CMS, Department of Health, HIPAA, and The Joint Commission.
- Case Management / Utilization Review skills including Admission Criteria, Benefits Eligibility, Care Coordination, Discharge Planning, Needs Assessment / Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case management.
Job Description :
- Perform discharge planning and case management.
- Coordinate patient care and manage the plan of care.
- Conduct needs assessments and order Durable Medical Equipment (DME) as needed.
- Review and ensure benefits eligibility and pre-certifications.
- Handle prior authorizations and manage worker’s compensation cases.
- Work with geriatric, ICU, heart and vascular, general surgery, general medicine, and MICU patients.
- Work every 5-6 weekends with one shift.
- Maintain a professional appearance; business casual attire required, no scrubs.
- Ensure compliance with work policies, including mandatory flu vaccination without exemptions.
Additional Information :
- Orientation is 28 hours and is 50% non-billable.
- Modules are to be completed pre-start and during orientation, varying by specialty.
