Summary : Women's Substance Abuse Services Clinical Case Manager will provide individual and group counseling to residents.

The Clinical Case Manager will carry a caseload of up to eight clients. This individual's clients will be primarily dual-diagnosed or individuals who have been identified as high-risk clients.

Schedule : Sunday - Thursday 12 : 00 PM - 8 : 00 PM (40 hours)

Essential Duties & Responsibilities :

  • Complete Clinical Intake within the first week of admission
  • Complete Treatment Plans within first 2 weeks of admission and update as needed
  • Be proficient in the use of the electronic Medical Record for each case
  • Meet weekly with assigned individuals
  • Participate in Clinical Supervision
  • Input entries into the Medical Record must be in line with the service line procedure
  • Participate in the on-call Rotation
  • Expand knowledge and participate in training
  • Understand DPH regulations and CARF standards as applicable
  • Provide transportation as needed
  • Establish contacts with DCF, Guardian, doctors and therapists etc. to involve all parties in the treatment process
  • Make outside referrals as needed
  • Attend weekly staff meeting
  • Provide weekend holiday and emergency coverage
  • Other duties as assigned

Required Education / Experience :

  • A minimum of a High School Diploma or GED is required
  • Bachelor's degree Preferred or clinical licensure preferred
  • A minimum of five years experience in the field
  • Individual and group counseling experience
  • Must have an updated MA driver's license

Required Skills / Knowledge :

  • Strong written and verbal skills
  • Ability to maintain records accurately and efficiently
  • Capable of responding to crisis situation
  • If recovering, two years of continuous sobriety required
  • Ability to work independently as well as part of a team
