What you'll do

Provide in-home physical therapy services to adult and geriatric patients under physician orders

Perform comprehensive assessments and functional evaluations; develop and update individualized plans of care

Coordinate with an interdisciplinary team to align therapy goals with the total plan of treatment

Initiate PT programs and teach family / caregivers specific techniques for safe carryover at home

Document timely, accurate visit notes in the EMR; communicate changes in condition promptly

Prepare clinical / progress summaries; collaborate on safe discharge planning and continuity of care

Adhere to agency, state, and federal regulations and professional standards of practice

What we offer (Part-Time / PRN)

$75–$100 per hour (experience-based) + mileage reimbursement between patient visits

Service area : Monterey County (Monterey, Carmel, Pacific Grove, Seaside, Marina, Salinas, Pebble Beach, Carmel Valley) and nearby Hollister & Gilroy

Work location : On the road / patient homes

What you'll need

Required

Bachelor's or higher in Physical Therapy from an APTA-accredited program

Current, unrestricted California PT license

2+ years professional PT experience

Current CPR certification

Valid driver's license and reliable smartphone for EMR use

Preferred

Home Health experience

Reliable transportation with auto insurance ; comfortable traveling within an assigned territory

Strong assessment, documentation (EMR), and communication skills; able to work independently in the home setting
