The Licensed Vocational Nurse (LVN) works to support the mission and vision of Neighborhood Healthcare by assisting Registered Nurses to support patients more efficiently.
This role will verify and document immunization records and coordination with the RN Immunization Coordinator. In addition, this role will assist RNs with triage and direct care to patients on an as-needed basis.
Please note that this position will predominantly focus on supporting the immunization program, with direct patient care duties performed on an as needed basis.
Responsibilities
Immunization Program Support(primary duties)
- Reviews and compares needed records and abstract data to CAIR, eCW, and patient’s immunization records; including identifying when patient immunizations are due
- Verifies immunization orders with RN for accuracy, such as correct vaccine, dosage, expiration date, and age
- Provides vaccine information statements (VIS) and administers vaccines, as ordered on pediatric and adult patients.
- Documents vaccines administered in CAIR, eCW, and patient vaccine records
- Sets up recall appointments for upcoming due vaccines
- Ensures all forms are kept up to date, such as Vis forms
- Completes required reports for RN to review
- Manages vaccine inventory to maintain accurate inventory, manage supply expiration dates, and rotation of stock to avoid waste
- Places vaccine orders in a timely manner to prevent running out of vaccines or creating an over supply
- Performs routine cleaning standards to maintain refrigerators and freezers for vaccines
- Conducts proper handling, storage, and management of vaccines in compliance with Vaccines for Children (VFC) / Vaccines for Adult (VFA), (BAP) Bridge Access program, and State County guidelines
- Provides staff immunization trainings in conjunction with Medical Assistant Trainings, as needed
- Provides periodic vaccine updates, in-services, or information to staff, as needed
- Monitors and records refrigerator and freezer temperatures at least twice a day
- Assists vaccine excursions in alignment to standard protocols
Direct Patient Care(as-needed basis)
- Assists with walk-in patients including taking vitals, obtain history of presenting illness, and report anything out of range
- Monitors the RN resource schedule of appointments, walk-ins, and the status of patients in the waiting room to identify high to low acuity of care;
assists RNs by taking care of less acute patients with RN supervision
- Assists with medication administration, including injections
- Performs EKGs on patients, including pre-op
- Keeps RN Triage room stocked
- Conducts monthly medication inventories and rotates stock based on expiration dates
- Assists with routine RN TEs, under RN supervision
- Performs labs prior to rooming walk in patients, as needed
- Helps to troubleshoot and address patient needs with RN supervision
- Assists with language interpretation, if needed
- Documents patient care according to standard procedures
- Records and reports observed symptoms, reactions, treatments, and changes in patient conditions to Provider and RN
- Assists RN or Provider with treatments, procedures, and special tests, as needed
Qualifications
Education / Experience
- Valid CA LVN license required
- Valid BLS certification from approved American Health Association provider required upon hire
- One year of LVN experience required
- Experience working in an ambulatory clinic and / or community health setting preferred
- Bilingual (English / Spanish) is strongly preferred

- Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with basic nursing procedures, nursing skills
- Ability to work in a faced paced busy clinic environment
- Ability to successfully manage multiple tasks simultaneously
- Excellent planning and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner
Pay Range : $31.52-$37.93 per hour, depending on experience. This role is eligible for a $2,500 signing bonus.
