Shift : 9 PM - 7 AM, Nights / Rotation : 7 on / 7 off (Rotation starts on Wednesday nights and ends the following Wednesday morning)
Guaranteed Hours : 40 hours per week
Experience : 2 years of hospital experience required
License : TSBP Registration required; Pharm D required
Certifications : Pharmacokinetic Dosing
Must-Have :
- Experience with Cerner (EHR) and Pyxis (ADM)
- Acute Care / Hospital Experience
- Neonatal / Pediatric Experience preferred
Job Description :
- Review and process medication orders using the hospital information system and pharmacy computer system to ensure appropriateness in terms of dose, dosage form, route of administration, frequency, duration of therapy, and patient’s disease state.
- Supervise pharmacy technicians and interns to ensure pharmacy products are prepared and distributed accurately, with the appropriate quantity and at the required time.
- Prepare and distribute pharmacy products accurately, ensuring appropriate quantity and timely availability, when necessary.
- Ensure all medication orders are appropriate for age, weight, and disease state.
- Review medication orders for potential allergic reactions, drug interactions, therapeutic duplication, and potential drug incompatibilities.
- Ensure PRN medication orders contain indications and are documented consistently.
- Document appropriate rationale for medication orders outside the normal range in the pharmacy intervention record.
- Provide drug information and educational programs to healthcare professionals and patients.
- Consult with physicians, nurses, and other healthcare professionals regarding dosage, dosage form, availability, proper administration, side effects, and possible drug interactions.
- Advise patients and families on medication use, including name, description, route, dosage form, and duration of therapy, among other aspects.
- Provide training to new pharmacy staff as assigned, ensuring compliance with training goals and objectives.
- Ensure compliance with all relevant regulations and guidelines.
