JOB SUMMARY

AtHouston Methodist the Pharmacy Clinical Specialist position isresponsible for performing advanced clinical pharmacy dutiesenabling the delivery of optimal pharmacotherapy to patients andfacilitating safe efficacious and pharmacoeconomically rational useof medications in the population(s) served.

This involves dailyreactive and targeted proactive medication regimen reviews forpatients within the designated area(s) of responsibility orpopulations served.

The Pharmacy Clinical Specialist supports thedelivery of pharmacy managed protocols formulary processes andmedication reconciliation activities while providing education tofellow pharmacists nurses and medical staff.

Additional dutiesinclude precepting pharmacy students and pharmacy residents onclinical patient care rotations within areas of responsibility andcontributing to fulfilling the organizations mission to provideinnovative personalized costeffective pharmaceutical care in aculture dedicated to quality andsafety.

Serves as arotation preceptor for students from affiliated colleges ofpharmacy and PGY1 or PGY2 pharmacy residents as well as being amentor for staff pharmacists supporting clinical skill advancement.(EF)

Processes all orders / requestsaccurately and efficiently verifying appropriateness of all drugsin reference to the patients diagnosis allergies height and weight.

Exercises good judgment in patient care decisions per performancestandard and documents patient care activities and patient outcomesimpacted by pharmaceutical care and clinical interventions.(EF)

  • Provides patientspecific and nonpatientspecific drug information support regularly.(EF)
  • Serves as a member on one or more MedicalStaff Committees and actively contributes to accomplishing thegoals of the committee.(EF)

QUALITY / SAFETY40%

  • Collaborates with other healthcare professionals to manage patients drug therapy for maximaleffectiveness while minimizing side effects adverse drug reactionstoxicities drug interactions and incompatibilities usingestablished internal and external guidelines.(EF)
  • Provides pharmaceutical care at thefundamental to advanced level for patients within the designatedpractice area(s). Activities include but are not limited toproactive drug therapy reviews and monitoring patient counselingtransitions of care management including medication acquisitionprocess support and management of pharmacistbased dosing protocols.(EF)
  • Participates in quality managementinitiatives such as but not limited to : medication use evaluationsadverse drug reaction and medication error reporting clinicalinterventions medication center reviews and inventory managementdocumentation and reporting.(EF)

  • Contributes to the provision ofpharmacoeconimically sound medication therapy by upholding HMFormulary standards in area(s) of responsibility and avoidingmedicationrelated complications.(EF)
  • Selfmotivated to independently managetime; effectively prioritizing tasks for self and for others withinthe specialty area of focus.(EF)

GROWTH / INNOVATION10%

  • Presents inservices for pharmacymedical nursing and other staff members in area(s) ofresponsibility. Develops educational materials and provideseducational programs for the pharmacy staff and other hospitalpersonnel. (EF)
  • Seeks opportunities to expandones learning beyond fundamental competencies with a focus oncontinual development as a Clinical Specialist.(EF)
  • Contributes to the design analysis andreporting of quality improvement programfindings.
  • Participates in research projects toadvance organizational quality of care and the pharmacy profession.Submits research and scholarly work for publication platformpresentations and / or posterpresentations.

EDUCATIONREQUIREMENTS

o Graduate of education programapproved by the credentialing body for the required credential(s)indicated below in the Certifications Licenses and Registrationssection

EXPERIENCEREQUIREMENTS

o Must have one of thefollowing :

o Successful completion of anASHPaccredited postgraduate year one (PGY1) residency trainingprogram or equivalent experience (i.

e. three years of pharmacyexperience per residency year) required; ASHPaccreditedpostgraduate year two (PGY2) or fellowshippreferred

o Three years of clinical practiceexperience

o Meets HM standards of anASHPQualified Residency Preceptor within one year ofemployment

CERTIFICATIONSLICENSES AND REGISTRATIONSREQUIRED

o Texas Pharmacist Licenseor eligible for Texas Pharmacist License; must obtain licensurewithin 90 days of employment

o American HeartAssociation Basic Life Support (BLS) certification within 90 daysof employment

o American Heart AssociationAdvanced Cardiovascular Life Support (ACLS) certification within 90days of employment for pharmacists regularly assigned to respond tocode blue events (See manager for details and trackingcertification information)

o American HeartAssociation Pediatric Advanced Life Support (PALS) within 90 daysof employment for pharmacists regularly assigned to respond to codeblue events (See manager for details and tracking certificationinformation)

o Neonatal Resuscitation Program(NRP) certification and training within 90 days of employment asdetermined by entity need (See manager for details and trackingcertification information)

o Texas PharmacistPreceptor Certification within 90 days ofemployment
