As a Remote Maternal & Child Health RN Case Manager, you will be responsible for providing support and addressing barriers to care and social determinants of health (SDOH) for high-risk pregnancies and NICU babies.
This includes delivering evidence-based trimester-specific and postpartum education, conducting mental health and substance use disorder screenings, and coordinating with obstetric and other providers for appointment assistance.
This position requires travel within the assigned region, as well as the ability to work from a designated quiet workspace in your home.
Primary Responsibilities :
- Provide evidenced based education to promote healthy pregnancy, delivery, and postpartum outcomes
- Create a positive experience and relationship with members
- Help members set goals and develop care plans
- Proactively engage members to manage their own health
- Perform outreach to providers, hospitals, members, and clinics
- Attend community events
- Travel within the assigned region as needed
Required Qualifications :
- Current, unrestricted independent licensure as a Registered Nurse in Kansas
- 2+ years of maternal and child health experience
- 1+ years of experience with MS Office
- Ability to travel up to 75% of the time
- Access to a designated quiet workspace in your home
- Live in a location with high-speed internet
Preferred Qualifications :
- Case Management experience
- Background in Managed Care
- Located in the state of Kansas
