Roles & Responsibilities
- The Inpatient Care Coordinator as part of the interdisciplinary team directs and coordinates the care of patients from their admission through transfer or discharge to achieve optimal resource utilization and discharge planning outcomes.
- The ICC performs comprehensive assessments to determine patient health care and discharge needs, problem solves and coordinates care to ensure the patient's discharge plan is consistent with their clinical course, continuing care needs, and covered services.
- In collaboration with the multidisciplinary team, internal and external disciplines to ensures safe and seamless patient transition across the continuum to prevent readmission.
- The ICC leads the interdisciplinary team in developing complex discharge plans with the intention of cost effective utilization of hospital resources, minimizing out of pocket expenses for patients, and patient advocacy
QUALIFICATIONS
- BLS and ACLS
- National Institutes of Health (NIH)
- Sound knowledge of anatomy, physiology, pathology, and medical terminology.
- The ability to stand for extended periods.
- Strong technical skills.
- Knowledge of medical terminology
- Passionate about patient care
- Requires a minimum of three years of current RN Case Manager experience.
- Graduate from a fully accredited RN program
- Active unencumbered RN Massachusetts state license
- One year of Cerner experience
- ARRT Certification
